I recommend that you use a tri fold folder for the driver packets (pic below), I am going to layout how I set my packets up, but whatever you choose to use your packets must include at least what is listed here.

There are other things that can be used as part of the packet, for example in my unit we have both a DWI pledge and Driver Pledge. I have included copies of both in case you would like to use them as well but they are not a required document.
The Driver pledge covers responsibilities of the operator and this way they read and understand what their responsibilities are as an operator. We have them sign the pledge and then the Company Commander signs it as well. We do this at the same time as the commander’s interview.
